Rare Pseudomonas Infections in Necrotizing Fasciitis

Monomicrobial Necrotizing Fasciitis and Sepsis Caused by Pseudomonas aeruginosa and Pseudomonas fluorescens: a Series of Ten Cases

Chen JL, Huang TY, Kuo LT, Huang KC, Tsai YH. Japanese Journal of Infectious Diseases. 2022;75(6):554-559.
DOI: 10.7883/yoken.JJID.2022.087

This retrospective series examined ten patients with necrotizing fasciitis caused by Pseudomonas species. Two patients died. The study describes clinical presentations and laboratory differences between survivors and nonsurvivors, highlighting the seriousness of this uncommon infection. Its small sample limits how broadly the findings can be applied.

Understanding MRSA in Joint Replacement Infections

Characterization of Methicillin-Resistant Staphylococcus aureus Isolates from Periprosthetic Joint Infections

Chen JL, Huang TY, Hsu WB, Lee CW, Chiang YC, Chang PJ, Peng KT. Pathogens. 2022;11(7):719.
DOI: 10.3390/pathogens11070719

Researchers analysed 36 MRSA isolates from patients with periprosthetic joint infections, comparing genetic types, antibiotic susceptibility and biofilm formation. ST239 isolates showed greater biofilm formation and resistance to several antibiotics, alongside more pronounced inflammatory findings in affected patients. The results describe local bacterial diversity and support further study of genotype-informed infection management.

Subsequent Fractures and Mortality in Taiwan

Incidence of different types of subsequent fractures and related mortality in Taiwan

Chen JL, Tai TW, Chou CY, et al. Archives of Osteoporosis. 2022;17:55.
DOI: 10.1007/s11657-022-01098-6

Using Taiwan's health insurance data, this study followed adults aged 50 and older after an initial fracture. It compared subsequent fracture patterns and associated mortality across fracture sites. Patients whose first fracture involved the hip had the highest subsequent-fracture incidence, underscoring the importance of secondary fracture prevention. The observational design identifies associations rather than proving causation.

Pathogens and Risk Indicators in Necrotizing Fasciitis

Bacteriology and mortality of necrotizing fasciitis in a tertiary coastal hospital with comparing risk indicators of methicillin-resistant Staphylococcus aureus and Vibrio vulnificus infections: a prospective study

Tsai YH, Huang TY, Chen JL, Hsiao CT, Kuo LT, Huang KC. BMC Infectious Diseases. 2021;21:771.
DOI: 10.1186/s12879-021-06518-5

This prospective study followed 184 patients with necrotizing fasciitis at a coastal hospital. It compared pathogen profiles, clinical features and outcomes, including differences between MRSA and Vibrio vulnificus infections. Shock and bloodstream infection were more common among patients who died. The findings emphasise that infection patterns and risk indicators vary by organism and clinical setting.

Immune Regulation in Experimental Joint Infection

GMI, an Immunomodulatory Peptide from Ganoderma microsporum, Restrains Periprosthetic Joint Infections via Modulating the Functions of Myeloid-Derived Suppressor Cells and Effector T Cells

Peng KT, Chen JL, Kuo LT, Yu PA, Hsu WH, Lee CW, Chang PJ, Huang TY. International Journal of Molecular Sciences. 2021;22(13):6854.
DOI: 10.3390/ijms22136854

This preclinical study investigated how GMI affects immune cells in experimental Staphylococcus aureus joint infection. Laboratory experiments and a mouse model showed changes in suppressor-cell accumulation and T-cell activity. These findings suggest a research direction for infection treatment; they do not establish that GMI is an effective or approved treatment for patients with joint replacement infections.

Looking Beyond the Hip in Recurrent Joint Infection

Recalcitrant Periprosthetic Joint Infection With Iliac Fossa Abscess After Total Hip Arthroplasty

Peng KT, Huang TY, Chen JL, Lee CW, Shih HN. Research Square. Posted January 21, 2021. Version 1.
DOI: 10.21203/rs.3.rs-144908/v1

This preprint describes seven patients with iliac fossa abscess identified during a review of 256 hip prosthetic joint infection cases. It examines imaging and combined surgical approaches used to address infection extending beyond the joint. The findings are preliminary, based on a small uncontrolled series, and should not be presented as a peer-reviewed treatment recommendation.

Plate Design and Support in Shoulder Fracture Fixation

The outcome of surgical management of proximal humeral fractures using locking plates: comparison between locking plates with different geometry

Chen JL, Chang FC, Lin SJ, Chuang PY, Peng KT, Huang KC, Huang TW. Journal of Shoulder and Elbow Surgery. 2018;27(12):2159-2166.
DOI: 10.1016/j.jse.2018.05.033

This study compared two locking plate systems in 100 patients with proximal humeral fractures. Radiographic impaction differed between plate designs, and medial calcar support influenced outcomes. However, functional scores did not differ significantly between the groups at the twelve-month assessment. The findings distinguish differences in fixation behaviour from differences in patient function.

A Vascular Emergency Complicating Hip Joint Infection

Pseudoaneurysm rupture with hemorrhagic shock in a patient with periprosthetic hip joint infection: A case report

Chen JL, Yang TY, Chuang PY, Huang TW, Huang KC. Medicine. 2018;97(26):e11028.
DOI: 10.1097/MD.0000000000011028

This case report describes life-threatening bleeding from an external iliac artery pseudoaneurysm in a patient with chronic hip prosthetic joint infection. Temporary control using an antibiotic-loaded cement spacer allowed subsequent vascular stent-graft repair. The case highlights a rare complication and multidisciplinary management; a single successful case cannot establish a standard treatment for all patients.
